Waivers to be completed for practice

Attached is the waiver required to use the pool at Durbin Amenity Center, this form is required to be returned before try outs begin. Please email back to [email protected] the signed form before your swimmer's try out.
There will be reminder emails sent out over the next couple days, if you know you have completed it, please disregard.
Please note, we only need page 3 signed and sent back to us, ASAP. We do not want to turn swimmers away for their try-out!
